Coaching Waiver

By entering your name below you understand and agree to the following:

Daniel Sunshine’s coaching services are not a substitute for mental health or medical care and he is not a mental health or medical professional. If you need mental health or medical services please seek out a professional. Daniel's coaching is not meant to diagnose in any way and is not a substitute for counseling, substance abuse treatment, therapy or any form of mental health care.

• I understand that Daniel Sunshine is not licensed by any state and that he is not acting as a mental health counselor or medical professional.

• I understand that the use of technology is not always secure and I accept the risks of confidentiality in the use of phone, text, email, and other technology.

• I understand that coaching and or products related to coaching is not a substitute for counseling, psychotherapy; psychoanalysis, mental health care or substance abuse treatment and I will not use it in place of any forms of therapy.

• I understand that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association.

• I understand that information will be held as confidential unless I state otherwise, in writing, except as required by law.

• I understand and agree that I am fully responsible for my physical, mental and emotional well-being during my coaching calls, including my choices and decisions.

• I hereby release, waive, acquit and forever discharge Daniel Sunshine, any agents, successors, assigns, personal representative, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by Daniel Sunshine as a result of the advice given by Daniel Sunshine or otherwise resulting from the coaching relationship contemplated by this agreement. I further declare and represent that no promise, inducement or agreement not expressed in this agreement has been made to me to sign this agreement. This agreement shall bind my heirs, executors, personal representative, assigns, and agents.

 

Cancelation Policy:

Cancelations less than 24 hours before the session start time will be charged a $50 fee.
